Cisco CME problems

All,
I am trying to help a friend out with a Cisco CME problem.
I do not have the configs to post at this moment, but the situation/problem is:

1: VOIP users can call out, but incoming Calls in the queue can not be answered.

2: Disconnect happens at a 4 minute interval when on hold.

Any insight would be appreciated.
 
How are calls coming into the system?
Does an AA answer or do they DID's to the extension?
Are there translations needed?
