Cisco 6509 freezing 1

I have 2 6509's each with 2 - SUP1+mfsc in single router mode. The primary sup card in the 2nd switch locked up and froze both switches completely. Had to remove the Sup card completely to get the switches back. Sho Tech pointed to lots of pinnacle errors. has anyone seen this before

Pinacle errors are normally associated with cards not being inserted fully. Check there are no bent pins on the backplane and when you re-insert the module make sure it is fully inserted with the levers.
