CICS burning hot

I have never had this happen before so I was wondering if this has happened to any of you. I was testing an older CICS and had it running for a few hours on my test bench. I started to smell a combination of wood and plastic burning. I lifted up the the CICS and my finger got burned as though I had touched a red hot stove top burner. It was from the heat sink on the back of the CICS.

Of course I unplugged it and opened up the housing. I noticed pat of the inside had started to melt. The small square power supply board was red hot! I bet this thing could have caught on fire.

Has this ever happened to you before?

P.S. Could the large CPU board be causing this problem or could it just be the power supply is bad? I have a spare power supply from another unit and even thought this happened the system was still on line and working during the test.

Replaced the power supply using the same CPU and it is running at normal temp.. Must have been just a bad PS.
