We have two domains and would like to go to one. How would I go about converting a primary domain server in one domain to a application server in another domain?
 
By "primary domain server", do you mean a Domain Controller? If so, you'd have to demote it, remove it from the domain, and add it to another domain. OR, you could create a trust between the two domains, and use ADMT to move things over.

The server in question is the only domain controller for the domain CAD and I would like to make it a application server in the domain STD. How do I demote it first and then add it to another domain. I have done this with workstations but never a server.
 
You've never demoted workstations, since they're not Domain Controllers.

Keep in mind that as soon as you demote the domain controller from CAD, the CAD domain ceases to exist. All user accounts, permissions, etc. are GONE.

The process of demoting domain controllers is well documented and available via search.
