CGI query

Appreciate some help - I am trying to upload files from MYSQL using SQL query defined string, that can be entered and process it according to the query. I know I need the METHOD=POST request, not sure how to code it.

<form action="/cgi-bin/script.cgi" method="post" enctype="multipart/form-data">

That enctype will make the file upload fields send the file's data to the "script.cgi" (to upload the file)
