Centrex lines not completing calls

I have a 3300MxeII with a Nupoint attached. 4 digit Centrex Lines at the villas used to be able to 4 digit dial the resort. Now 3 out of 5 attempts end up with 1 or 2 rings in the caller's ear then "Dead Air" until you hang up.
Their appears to be a hunt group with single line phantom IP phones in the 3300 that is suppose to be doing something with the calls. The "Traces" in the 3300 Maintenance Commands show the calls being handled by this hunt group 4900 then to a member in that group like 4901. The members are labeled "4901 MIAUDIO 1"? "MIAUDIO 2" 3 and so on for the 8 members.
When you finish dialing the 4 digit extension you want to call, and you hear "music on hold" for just a split second you know that your call will complete.
Remember that some calls do go through, but most do not.
The 3300's software is 4.0 and the nupoint is 1 lower that the latest version just out.

Have you got any other mitel applications?
e.g. a wakeup server?
Having miaudio ports usually points to some type of IVR app these are just software IP ports maybe some have shutdown and need restarting as they are usually windows TAPI/WAV which uses both Mitel and Windows ports and they can get in a intermidiate state.
restarting them will often put things right.

Just my thoughts, I could be wide of the mark
