I am having the same issue. Did you find out what is causing this?

I have Nortel working on it but no word yet. I will post an update if they are able to fix it.

I would say that you are running SU03 line-up?
When and how many times does this happen?
What CCT Client API are you using????? Who developed it?

Nortel did finally figure out what was wrong, see the resolution below. The difference for me was it was persistent, didn't matter how many times we rebooted which server the issue never went away until we followed the steps below. The tech said its possible it will be fixed in a later patch.

4.7.1.5
Login failure following CCT Restart
Problem
Related CR: Q01473932
Details: An intermittent problem has been detected where Agent logins initiated from CCT
applications (including Reference client and CCAD) fails after a CCT Server restart. The
problem has also been observed following restart of either of the NCCT TAPI Connector
Service or NCCT Server services on the CCT Server.
Workaround
When this issue is encountered, a restart of the CCMS OAMCMF_Service on the CCMS
server resolves the issue. NOTE: CMS OAMCMF_Service service may be restarted during
normal operation without impacting CCMS voice call-processing.
