Last week my office attemped to upgrade from CCM 3.3(3)sr4a to version 4.1(3) with no success. Half way through the installation we got a following errors at the same time:
Error 1
Title: Cisco Call Manager Installation Information
"UMX.dll failed to register. After you complete the installed review the log file."
Error 2
Title: tomcat.exe = entry point not found
"The procedure entry point ?ClearODBCStatement@CConnectionHolder@@QAEXXZ could not be located in the dynamic link library DBL.dll."
We clicked ok on both and the install completed. When the Publisher rebooted everything appeared to be ok until we realized we could not access the CCM Admin page. We would get prompted for logon and PW but after we enter the information we got the following error:

Microsoft JScript runtime Error '800a01ad'
Automation server can't create object
/CCMAdmin/includes/verify.asp line 99
This install was conducted with the Cisco CCIE Voice assigned to our account onsite and the only info he found was to run a script from the command prompt that didn't work or we that had bad CDs. The TAC engineer stated the same.
Sound familar to anyone?
To add insult to injury after the second install failed the mirrored drive with the good image got corupted and we had to rebuild the publisher form sratch.
