Cause of - "the number you have dialed has not been recognized" ?

Customer of mine reported this issue and I cannot seem to figure out why it's happening, all the settings seem to be in order, any ideas?

Customers report:
When you call the extension the caller hears "the number you have dialed has not been recognized" however the user claims his phone rings. If he picks up while that error message is playing/when his phone is ringing, they're not there and instead his phone will call back whoever called him.
 
That message isn't coming from any standard IP Office feature. We would need a lot more detail about the whole call routing, types of phones and lines involved, etc.

Stuck in a never ending cycle of file copying.
 
Check to see it it’s twinned.

look at SSA and see what is happening when you call the user.
 
sounds like his phone is twinned to a wrong number.
