carrier code on international calls

I have a new Int'l 6.3 standalone cm that just went online. the users dial 0 as the ars code and then 00 for the int'l code. Then, they use a carrier code (21) followed by the number they want to dial.. for instance to dial the US the string is:

0-00-21-1xxx-xxx-xxxx How can I have the system insert this 21 so the user can just dial normally (ex. 0-00-1xxx-xxx-xxxx)?
 
Both the route pattern and the ars digit conversion tables can be used to achive this.
