Can't put an inbound SIP call on hold

I'm connecting an IP Office 9.1 to a SIP provider through an SBC, but I can't put inbound calls on hold. I have 2-way audio in and out, and I can hold outbound calls, just not inbound. I have the WAN port of the IP Office directly connected to the A1 port on the SBC, and the B1 port to their router connected as a DMZ.
 
What happens when you press Hold then?

Nothing. It just beeps. Transfer, Conference, Park Buttons, all do nothing. If I connect B1 directly with a public IP, no problems. They don't have a spare public IP though. Turning SIP ALG on their router off, gives me 1-way audio, so I leave it on.
 
Why do you need a SBC when it's already NATd behind a router/firewall? It's not going to do much but break things and complicate matters :)

It has certainly done that. I suppose mainly because the sales guy was able to sell it.
