Cannot log into Unity Web app? What does this mean?

After Loging in I get this

Missing live session flag.
For some reason, /web/global.asa did not fire.
Check that this file is present.
This problem could also be related to DNS issues.
Cannot continue.

What do you recommend. Thanks

I believe I had to run the grant unity access command for each user but this was a couple flavors ago. What version are you running?
