The password for the MDUSER account was changed using IOUAC
I can no longer log in via telnet on ANY of the accounts

Did you try the serial port(s)?
If telnet is not working, then an advice could be to try the TTY / serial port.

if the change is not yet dumped, you can reload the PBX(push/release LPU button) to undo the change... please note that you'll also loose all changes that were made since the last dump.
