Cannot get form to vaildate in XHTML 1.0 strict

Dear Web Gurus,

I have a form on my website that gives me about a thousand errors when I try to validate it:-


I have been a good boy and avoided using tables, however I'm not an expert in forms and I'm clearly doing something very wrong.

Any help would be greatly appreciated :)

I get 7 errors and a load of warnings but I don't understand what is wrong with the page.

Can anyone explain what is wrong with this tag?

Also, when viewed in IE6 the bottom of the form drops out of the border....
 
There's nothing wrong with the tag itself, it's where it is located within the page. Read what the error says:
document type does not allow element "input" here; missing one of "p", "h1", "h2", "h3", "h4", "h5", "h6", "div", "pre", "address", "fieldset", "ins", "del" start-tag.
