Cannot format a floppy disk

When I try to create an Emergency Repair Disk, I receive a message that setup could not format the floppy.

When I try to format the floppy, I receive the following message - "Error in IOCTL call".

I tried several different disks with the same results. These same disks can be formatted on other computers.

I am using NT Workstation 4.0 SP3. [sig][/sig]

Can you read data from floppies? If not, try checking the cabling / replacing the drive.

Are you working with an HP Server? HP recently released tech-notes/fixes for some of their servers and their floppy drives. [sig][/sig]
 
It is not an HP server. It is an Industrial Computer Source computer. [sig][/sig]
