Cannot create new phoneset from TM 3.1

Get the following when trying to save phoneset in TM 3.1:

ERROR: duplicate key violates unique constraint "telephone_pkey"

Trouble is, have seen it before, dont remember when - anybody out there with ideas??

I have also seen this when trying to use templates created in earlier versions of TM.

I usually out the template, then rebuild, and am good to go.
