Can Someone help with an email form? 1

I was wondering if anyone knows where I can find a template form made in asp. I need it to email the results of the form to a specified (internal to my Database ) email address. I am trying to have a template that I can create and recreate in the form of a test in which the results are emailed to a person's coach at submit.
