Can not dial extension from IVR

Anyone no what would stop me from dialing any extension from the IVR I get the message this is an Invalid Extension?
The IVR Direct Dial Option is set for ALL EXTENSIONS
You can call between and transfer to extensions with no issues. You can make an option in the menu to go to that extension and it will go. But if you take that menu choice out and just try to dial the same extension you had in the menu you get the message INVALID EXTENSION.
 
Is this for all your extensions or a specific extension. Reason I'm asking is because I had this happen on an extension and I had to delete extension and then add it again to extension list.

the guys at emetrotel helped, it turned out to be the dtmfmode on the SIP trunk needed to be changed.
