so you want to stop spam mail at the ISP level so that you don't have to download it?
if you need to do this then look at getting someone else to filter your messages on the net, there are a fair number of free email places that filter spam, you could use that as your address, receive from there (send via your old smtp, but use a reply-to to the new filtered email) and you should have no trouble with spam.

well the sendmail server can do the spam scanning stuff anyways ... there are sendmail links in the page i gave you above ... and i've got hold of a scanner that does virus checking (if you have it) as well as spamassassin called (imaginatively) "mailscanner" ...
