Calls some times go to the wrong extensions

we have a merlin legend phone system (408 board wiith mlx-10p phones ) with inuity audix. Often someone will enter an extension into the AA ( auto attendant ) and it will transfer them to a totally different extension. I have wittness this myself.

I checked the configurations of the phones and they appear normal. What else should I check?
