Caller ID not displaying on single extension, Stumped

Hey there, I have one staff member's caller display isn't working, when his phone rings it simply shows the line that is incoming to his extension, instead of the caller name/number like it used to. It only happens on one extension and I've tried clearing custom features, changing phones, and going through the manual. Any ideas to troubleshoot this would be greatly appreciated.

Phone Control is Panasonic TDA-30
Phone Desktop is KX-T667
