I am forwarding phone calls to my cell phone from a 5621 SW IP handset. The forwarding is working, but every number comes up as the PBX's main # on my cell phone and not as the original # that called. Is there any way to forward the # that called into the PBX?

No.

You cannot pass the cli of an incoming caller on an outgoing call.

Its against the law.

ACA - IP Office Implement
ACS - IP Office Implement
ACE - IP Office Implement
ACI - IP Office Implement

If you get the forward done at the exchange level, then it can work.

However, I don't know if the IPO supports ISDN forwards

If at first you don't succeed, skydiving is not for you.

If you do from Network Level then it works here in uk. This can be done with services such as * services or the likes.
You would need to speak with some one who uses the service to see if it sends the original CLI.

CLI is preserved, one of our customers does this with their ISDN2. However, it uses a different PABX and P-P isdn2 (rather than P-MP).
