Norstar 7.1 MICS on inbound calls to extensions with DIDs the caller does not hear ringing on their end. Call rings on the extension and is connected when the extension is picked up. Any programming or settings that could cause this?

This issue should be brought to the attention of your telco. The MICS simply tells the CO that the call is accepted. The CO is responsible for providing ringback to the caller.
It is the same thing if the call can't be established. The MICS only tells the CO that the call is rejected and why. The CO is responsible for providing the appropriate signal to the caller (for example, a busy tone, an overflow tone or a recording).
