Caller Can hear other calls while on hold

Have a strange issue, I've gotten numerous reports that when a agent, & non-agent places a call on hold, when they un-hold the call the Caller states they actual heard another conversation complete different floor/department.

FYI always External Calls that was reported, Im assuming my PRIs???

CM 5.2.1 (R015x.02.1.016 Patch 21656)
using 9611G

Got a G450 doing that while on 1 carriers PRI, switched to another, and moved to another building after. Getting it swapped.
