Call Tranferring on KXTD-1232

I have a KXTD-1232 & TVS-200 setup. On one extension I'm trying to get unanswered calls sent to an outside line to call a cell phone. (All others go to the voicemail system).
I've tried the deskset forwarding routine of:
-go off-hook
-press FWD/DND
-dial 6
-dial outside access code (9)
-dial phone no.
-dial #
I get the confirmation tone, dial tone & display on the deskset but it still won't transfer calls out.

Is there something I'm missing?

Do you have forward to line ticked/enabled/turned on in class of service settings?
 
There are 12 desksets on this system, all have COS1. I have both forwarding & transfer to CO enabled on this COS.
 
if you dial the ext that is forwarded to a co via
intercom what happens?fast busy ?
 
some other things that come to mind if the answer above
is no .what does your tvs ext transfer sequence
look like ? if it's not fxd then the tvs may nothand the call off properly to the ext we really need to narrow down the pbx or voicemail that is causeing your problem...
also I do remember that early versions of td and tvs
combo did not do remote notification or transfer off net
prperly!!
