call starts with double connection, one never terminates

Telecom provider called with a warning that a call was up for weeks. Accoording to their log the call started with a double connection to the same number. When the call was finished, one channel on the PRI was never released for over two months. In the customer callcenter application log the call was made from an extension which was in normal operation af that incident.
IPO 500 version 5.0.26 PRI. Does anybody have seen this before?
 
Was the destination number in the Carribbean?
