Call Pilot 150 DTMF Issues

I have a customer with a CP150 mini. They are having issues with transferring to extensions. At least 15-20 times a day a customer or employee of the company will call in, get the AA, dial "255" (example)and get a Invalid Extension" message.
Has anyone ran across this before?
 
what release is it? Nortel put out a couble of releases that had dtmf echo causing the system to not recognise the digits or transfer to the wrong extension.
 
I am not sure of the release. i will check that out this week. Thansk for your help!!
 
Make sure that the touch tone gate is not on.
