Hello,
I've set-up a T1-PRI switch-type NI2 trunk on my IPX2400, my DCH is up and all BCH have gone through Q931 "Restart". But when I dial the outside number (9xxx-xxxx) the call never makes it out of the trunk. Inversely, if the trunk receives an incoming call from the CO side the call falls dead at the trunk. The Q931 message sent by my NEC T1 on an incoming call is.

I have gone through my PRI config and my LCR config and nothing obvious pops up.

Has anyone seen this before?

Your cause code and location point directly to the layer 1 status of the B channels.
Have you confirmed they are in Make idle?
I'm not much of a 2400 guy but I have done this when I forgot to do MBTK. At least I think that's what the CM was.

Yes.
I've done MBTK and MBRT, both with no luck :-(
