Call Foward Profile problem on 5340 IP

When I try and activate a profile on the 5340 I get an error that activation failed. I have checked COS and it looks good. I followed the instructions and still get the same error over and over. Any suggestions would be helpful.Thanks
 
Just an update. I turned call forward on by using the feature access code and it works fine. But if I try to change the forward destination using the applications key I get the error (profile failed) again. What's strange is that I set up a new 5340 and pressed the application key,pressed call forwarding, then entered an ex # and it worked fine. But when I tried to change the forward destination a second time I got the error(profiled failed). It seems to me that there is a bug in the system.
 
I just enabled the HCI options in COS in our lab system and it worked. What's strange is that in the customer's system it was already enabled and they got the same error message.

No problem. When you access the site, check the Maintenance Warning logs, there should be a line created when you attempt to set the call forward:

"Please check COS configuration to make sure 'HCI' options are set for DN = xxxx"

You'll know then if the COS is right or not.
