Call Forwarding

I have a client running a IPO 412 2.1.24, and I am having an issue with their "Call Forwarding All" button. This is in a call center environment, and all the agents are being asked for a system security code when they press their call forwarding button. I've tried removing and replacing the buttons, but this is only a temporary fix. Any suggestions?
