This happened after upgrade to 9.0.
If I put CFNA on the extension to my cell phone (external number) and call this extension from my desk phone I see it ringing for CFNA timer long, then it places a call to external number and keeps ringing on extension AND the cell phone until cell phone VM will pickup.

If I call DID number of this extension I see the same progress with the same timers but when the cell phone VM picks up I hear the beginning of VM prompt for about 0.5sec and then call is being dropped.

I increased CFNA timer, so I can see that VM of the cell phone is answering faster then my timers would expire and still I get calls dropped pefixed with 0.5 sec of VM prompt.
Interesting additional observation, calling party is not getting busy or any other signals, just dead air.

If I pickup eather extension or cell phone before this timer expiration a call will be connected ok.

If I put Call Forward Always, the extension will not ring and all calls from extensions and DID will connect to VM of my cell phone.

My concern is that I'm about to upgrade a system which has this call forward feature enabled for more then hundred users and I will be smashed in my chair if this would happen to that site. So I need to define if it is a software issue of wrong programming.

9.0.0.41 is a bad version with a lot of bugs
Use 9.0UR2

While 9.0UR2 on its way. I'm just wandering if it's normal when original party would still ringing while call is being forwarded to another number. I tried it using re-routing forms and got the same issue. May be it's not a bug but a feature and I just don't know how to use it?

There was a new COS option added specifically to perform the way you are describing.

Maintain Ringing Party During Recall

This option determines whether an incoming trunk call that the attendant transferred to an unanswered extension (including Hunt, Pickup, and Ring groups) continues to ring the extension after the trunk's No Answer Recall Timer expires. When the option is set to "Yes" in the attendant's COS, the console and extension will keep ringing until one of them answers the call. When the option is set to "No," the extension is released (and stops ringing) immediately after the recall timer expires.

This option can also be applied to extensions.

Was set to "No" for extensions and trunks

Interesting detail. It works ok for internal call forward destinations. Calling party can be internal or external.\

One more. If I answer my ringing cell phone, the call will be connected. It happens when call is being forwarded to Verizon voicemail. I hear 0.5 sec or so of Verizon prompt and then dead air on callin party device. The call is not being dropped, there is no audio. Trunks are PRI connection without any issues.

Because this is a relatively new feature intoduction you may be encountering an unknown bug. An upgrade couldn't hurt and as tlpeter said earlier, 9.0.0.41 was not a good load and has several known issues.

An upgrade to UR2 fixed the issue, so I assume it was a bug.
