Call forwarding is not working. 1

I have time profiles programmed for closed hours, phantom user with forwarding number, and the calls are not forwarding. What am I doing wrong? We just had our main number ported over to ATT pri over MPLS. I was working fine on the temp number before the port, but didn't actually go live til the port was complete. Please help.
 
Have you done a monitor trace to see what happens?

If it did work before it sounds like the service provider doesn't allow you to send originating CLI.
