Call Forward to Voicemail

We have an AVST an NEC IPX. We have a number of users who accidently forward their phones for all calls to voicemail. Is this a common ID10T error? Or have we created our own problems? Any one else have this issue. Apparently we have been running this way for years. Someone will open up a ticket that all calls are going to voicemail and the reason is because unknowingly they sent all their calls to VM>

Struggling here as I don't know what an AVST is...
The most common problem we experience here in OZ is because the most common call forward all code is *1.

The major carrier here released a network feature for checking the number of the last missed call, the code for which is *10#. To add insult to injury, the line access code here is 0 (whereas yours is 9). So if anyone unwittingly dials *10# They effectively call forward all to an outside line and then dial #.

As you have already guessed this gets them nowhere and we get called in to deal with the fault that "when I dial extn XXX I get RST".

It's hard to know what your solution would be without a full understanding of the problem. If you could expand your description it would help.
 
it's a common error on a lot of larger sites.
I've used a few ways to stop it or at least help me diag it.

1. standardized CF ALL button on the phone. if they call in and its on it's an easy fix.

or

2. remove the call forward all access code and give those that need it a feature button.

3. leave the code and pull access from your repeat offenders.

out of all of these number 2 is my fave for solving this issue.

This is most definitely a fat finger dial issue.
The most common one I see is the caller ID call back or last number call back or simply someone going off hook trying to dial a number with out first dialing 9. The digits they dial match the call forward all code and presto the phone is forwarded.
Check your numbering plan and if no one is using the access code for this feature remove it. If you don't know if anyones using it delete it anyway and you'll soon find out. If someone does need an access code to do this make it something like **3 or similar. This will cut down on the errors.
