Call Forward Activating

One of our office locations has an 11c installed. About 1 week ago to present, phones are randomly "self activating" the CFW key to the number programmed in the key. Day of week or hour of day does not appear to be a pattern. There is no programming for redirection by time, day, etc in the switch and all the phones (3900 series) have CLS RTDD programmed. Has anyone seen this occurring and/or what might the problem/cause be?
