Call Disconnect After 6-8 Seconds

Hi Everyone, I have a problem here and hope you can help me with.
We are using CCM4.1.3, the problem is when we try to make an outbound call, I can see the call is connected to the other party but I can not hear anything and suddenly after around 6 seconds I here tone from the CCM and the call is disconnected.
I have tried to call my own mobile and here what I have,
1- I make the call from the IP phone and enters my code.
2- The IP phone shows connected and starts to count, and my mobile starts ringing.
3- On the IP phone I hear a unavailability busy tone and the call disconnects.
4- The call disconnects from my mobile.

So can any one help!
