Call being dropped and NU tone returned to caller. S/W logs produced

Have a Mitel 3300 MXe running MCD 7.0. Users are reporting calls cutting off on answer. Sometimes NU tone is returned to the destination (answering extn or console)

Calls are inbound on Embedded E1 ISDN30 or via DPNSS. Software logs show hundreds of sets of 3 logs as follows:

Log 1: Unexpected ResultprocessRequest - Task: CscRequest (iMiFsmEvent(id PROCESS_REQUEST)(type CscEventRequest)(request (CscRequest(Type CSC_OPEN_RX)(RX (Port(PortId 10)(CompId DPNSS_SP)(PortType PORT_TYPE_TDM_BUS)(GatewayId 1)(ConnectionResponseTime ))(Stream 1))))) Failed for connection (iMiFsmContext(RtpTdmConnection ((State (MiFsmState CONNECTED)))))

Log 2: Unexpected ResultOpenRx - Task: CscUnable to process this request, reject.

Log 3: Unexpected ResultHandleEvent() - No transition from (MiFsmState CONNECTED) on event (iMiFsmEvent(id CONNECT_REQUEST)(type CscEventOpen)(MsgId 2220033894)(ConnectionId 72548873)(Port (Port(PortId 10)(CompId DPNSS_SP)(PortType PORT_TYPE_TDM_BUS)(GatewayId 1)(ConnectionResponseTime ))(stream 1))(ReserveMany 1)(MDL(MediaDescriptorList(MediaDescriptor(MediaType audio:RTP/AVP)(StreamDirection recvonly)(IpAddr 255.255.255.255:65535)(CodecList(MDCodec(Codec TERM_CAP_G711_ALAW64)(AVProfileNum 8))(MDCodec(Codec TERM_CAP_G711_ULAW64)(AVProfileNum 0)

Initially it was thought that this was something to do with the recently installed MiVoice Business Consoles, but have now removed these and logs persist and other extns also still seem affected.

Any pointers in the right direction would be appreciated
 
Finally got to the bottom of this issue.

It turned out to be a faulty T1/E1 module. Replaced module and all these nasty logs disappear and no more call cut-offs
