"This special file contains design elements of server-based databases and cross replica journaling information." It is similar to the cache file that a web browser uses to increase performance.
So, if and when this Cache.dsk file grows to a large size, would it be advantageous to delete it? And if so, will Notes automatically recreate it? Or will you have to shut down and restart Notes?
Affirmative, you can delete cache.dsk as often as you wish (when Notes is not active). You might experience a (tiny) increase in load times for the first few documents you open after, but it will soon disappear.
In practice, I find this functionality rather useless and would prefer to have an option for forgetting it. Limiting it to 5Mb is quite enough.
