CA48 Adapter Pinout??

Hi!

I have a client with a CA48 Intercom Dialer. Someone 'needed' the adapter (the 304B splitter that injects the 48v DC that powers the unit) and they took it...

Does anyone know what the pinout for this splitter/adapter is?? If not, does anyone know where to get one?? (or if I even have the correct name/part number??) Avaya has been NO help on this issue!!
