Button to automatically create a calendar appointment

Is this possible?

I want to email out a note that has four date and time options, and have the recipients simply be able to click on the preferred date, and have it automatically added as an appointment in the recipients calendar.

You are taking the calendar function a bit too far.
The idea is that you first agree on a date, then you send the invitation.
Notes cannot do what you are asking for with standard code.
You could design a new form for that, but the recipients would need it too (or at least they would need to Allow Stored Forms in their mail db properties).
