building a reports

Hello
i am fairly new to access and don't seem to be able to get a report working.

What i have is a form with three listboxes.
A selection from list box A gives the options to list box B
A selection for List box B give the options to list box c
& A selection from list box c shows the contents in a textbox.

List box B and C are multiselect, and the listbox shows the field that relates to all the options that are selected.

I now need to take the three choices and put them in a report with the final report being all the content of the textbox.

i need it to look like below.

Choice from list box a
Choice #1 from list box b
Field from choice in listbox c
Choice #2 from list box b
Field from choice in listbox c
Choice #3 from list box b
Field from choice in listbox c
Choice #4 from list box b
Field from choice in listbox c


And so on. I would like to export this to word after.
can some one assist me.
