What causes a BPV error?????

In my experience BPV (bipolar violations) are usually caused by an AMI circuit being optioned as B8ZS.

Bipolar Violation (BPV) - The occurrence of two successive pulses of the same polarity in a bipolar signal. In T I, this is detected as an error

OLD ROLMEN WORKING ON NORTELS

i've seen more bpv errors, when they are not part of other errors crc etc, happen between the smart jack or csu and the card... common cabling, more then one t in a tie or patch cable, patch cables that are hand made, or spliced.. csu's that are 3 floors away... not all, but most of the repeative bpv errors fell into that block... bpv, with no other errors.. usually when you have a repeater or loaded cable from the co that is optioned wronge somewhere in the ckt, you'll see other errors... if a t is optioned esf on both ends and ami at center points, it will stay up, you'll see bpv's and usually slips, and/or crc errors... every case of trouble is different as is experience.. these are things i look far first, after 30 plus years i still get fooled every now and then...hope this helps
