Box type component or script

I am looking to create an effect that I saw once on a program, but I can't remember the program. What I want to do is when you click on a button in my toolbar I want that to popup a small well, popup, with some edit boxes and different things in it and for there to be an ok button to save and close the popup. I don't want it to be a form, I want it to be like a tooltip but on clicking with edit boxes in it. I saw a program do this once I just can't remember what it was. Any ideas?

use a form, change the BorderStyle property so that it doesn't look like a form, and position it to appear next to the toolbar button.
