Bm-12 giving incorrect information

I have an IPO release 9.0.3 and it has been installed for a few years now. The receptionist has a 9508 and 2 BM-12's and soft console. Recently, the BM-12's are showing users phones ringing or they are on a call. Soft Console and system status show that is not happening. When I unplug the phone to reset it, it works fine for a few days then goes back. I have upgraded the system from 9.0.3 to 9.0.12. I had 3 phones on the combo card that displayed "upgraded denied" one was the receptionist phone, swapped her phone with one that did upgrade, and the problem keeps happening. I have replaced both BM-12 thinking they might be bad. I have deleted the user profile and re-created it, I copied and pasted all the buttons back on, problem comes back. I have replaced the power brick and physically moved the receptionist phone from the combo card to the DS30 and the problem continues. Then I upgraded them from 9.0.12 to 9.1 and the problem comes back. I know this was a known issue back in the day, but I'm at loss on what to do next. Any suggestions would help. Thanks all.
 
Try deleting, merging and then recreating the user.

Check if the bin file for the BM modules is the correct version for R 10.1 it is "GBM32_B_1_1_0.hex
 
Intrigrant I just look at the system and GBM32_B_1_1_0.hex is present. Anything else you can think of?

Holdmusic34 I have already deleted the user account and recreated it, sorry if I didn't state that in the beginnging.
 
BM32 module firmware corruption.

Downgrade the firmware of 1616 phones along with BM32 to some previous load and then upgrade back to the same e.g. ‘1_2100.bin’ and ‘GBM32_B_1_1_0.hex’ load.
1. Downgrade the firmware of 1616 phones along with BM32 to some previous load.
2. Upgrade to the phone firmware to the same ‘1_2100.bin’ and BM32 module ‘GBM32_B_1_1_0.hex’ load.

What did you ended up doing to resolve this?

I'm currently having the same issue. I also have a 9508 phone with 2 BM 12's attached & just the first one (closest to the phone) acts up. If I disconnect the second one, the first one then works fine. Tried replacing the phone, the BM 12 & power injector.

I have a different Ext. with the same set up 9508 & 2 BM 12's & that one works all the time no issues at all.

I see the GBM32_B_1_1_0.hex in my bin file also.

Did you do what Holdmusic34 says? I'm confused because he starts talking about a 1616 phone.

I had this same issue when I had 4 9508 phones with 3 BM12 on each phone. The first console would always work, but the other 2 would go blank or show the wrong info. I tried upgrades and different consoles, to no avail. Unplugging the cable from the first BM12 to the 2nd BM12, would temporarily fix. The only way I got this fixed was to get a new 9508 and 3 new BM12, hook them together and make sure they are plugged in at the same time to the IPO. When the phone upgraded the BM12's seemed to upgrade with the phone. I think my initial problem was I only plugged the phone in with 1 BM12 to the IPO, and then added the other 2 BM12.

I later did switch the two consoles with each other & seen the problem stays with the same one every time. So, that was the direction I was going to next.
 
All, Sorry that I didn't post the next step that I did, just waiting, hoping it was it. I took a new out of the box processor and new SD card, up graded everything and reprogrammed it from scratched, no coping and pasting from the old config. The only thing that I imported was the licenses and the directory, everything else including buttons are new. It's been a few months now and haven't heard if the issues continue or if I have fixed them. We have recently added another site of IP phones to the mix, and added more user buttons to those button modules, so you would think that if the issue is still present, they would have said something.

Hoping I don't have to go through all of that & going to try what Skeeter1880 said. Ordered two new BM 12's today (have a new phone), then see what happens.

I also sometimes forget to post what solves the issue & will try to remember how I turn out.
