Blowing up 012 telephone cards

My system has just blown out a second 012 card. "it really smoked the same chip on both boards" I have a replacement third board, but I'm afraid it will blow too, The last one I replaced just 30 days ago. What could be causing these boards to blow?
 
How far out of the building are the extensions connected to this card?

 
Well, that trick question didn't work.

In the "OLD DAYS" there was a problem with 012 Ports that were extended out of the building getting ZAPPED by lightening and stuff.

"All on the same floor" would make me wonder about the POWER UNITS and even perhaps STATIC ELECTRICITY.

There may be another thing, but at this point I am at a loss.

Are they regular phones or modems or ???

 
Have you tried a different slot on the chance the first card damaged the backplane?

Last night I added the new card to a new slot in the second carrier, I left the bad card where it was in the first carrier. The system calls it a "5 T&R card"? I also removed any unsed connections to the new card, leaving only 4 vm ports, 1 fax and 1 conference phone. Maybe a short in a station cable could have been the cause. I'll see how long this card lasts.
