What might cause Blinking Message & Call Back button LEDs, and pulsing dial tone when there are no messages present in voicemail?

Any advice appreciated.
 
Maybe a "Silent message"?

Did you try any of these:
-dialing 368
-Leave yourself a message then try deleting again
-SPEED * * to delete all messages

I don’t think vm is setup on this station, It does have the common password given at time of installation, but the voicemail doesn't ever pickup when calling this extension.

I'm looking at section 3.10.4 (Station Message Wait/Call Back) of the manual.

Dialing 386:
- Is the 386 dialed after pressing Message/Call Back? (not in manual)

Leave yourself a message then try deleting again
- Could not leave myself a test message, vm doesn't pickup.

SPEED * * to delete all messages
- What is the button sequence to use Speed * * ? Is it: Message/Call Back, Speed * * ?

The message display on this phone is:

ST..CL..BS..VM..US..MS
001 000 002 000 000 000

Not sure of sequence (never heard of Summit systems until now).....but yes I would have thought just that.
You can have several types of messages and not just voice mail.

Looks like you have messages in all sections, 19 in total.

From the guide:

To retrieve SMS Messages:

1. Press [MSG] or [CALL BACK] button. The message contents summary will be shown as below.
[highlight #729FCF]ST[/highlight]__CL_VS_VM__FS_MS
[highlight #729FCF]001[/highlight] 001 005 006 001 004

2. Dial “6” to select MS, SMS message wait. (my note: or dial the number required to view other messages)
“1” => ST: Station Message wait,
“2” => CL: CLI Message wait
“3” => VS: Integrated Voice message wait
“4” => VM: external Voice Mail
“5” => FS: Feature server
“6” => MS: SMS message wait.
The display shows the first line of received SMS messages

3. Use the [VOL UP]/[VOL DOWN] to scroll through messages.
MESSAGE: [0] EMERGENCY CONFER
[1] LETS GO TO LUNCH

4. Dial the message number (“01”~“10”) to view the entire message, use the [VOL UP]/[VOL DOWN] to view each line of the message.
MESSAGE: [2] WE HAVE A MEETING A
[3] CONFERENCE AT 9 AM

To delete SMS messages

1. Dial “#”.
DELETE MESSAGE?
YES : 1 NO : 2 ALL:3
2. Dial “1” to delete the current message, “2” to return to idle or “3” to delete all received SMS messages.


My note: Example per above "2. Dial “6” to select MS, SMS message wait."
I wonder if you are to press 4 for VM then press # 3 to delete all VM's?
This would then be how to delete a particular section only:
Full sequence: Press [MSG] or [CALL BACK] button,press 1 for ST then press # 3
Full sequence: Press [MSG] or [CALL BACK] button,press 2 for CL then press # 3
Full sequence: Press [MSG] or [CALL BACK] button,press 3 for VS then press # 3
Full sequence: Press [MSG] or [CALL BACK] button,press 4 for VM then press # 3
Full sequence: Press [MSG] or [CALL BACK] button,press 5 for FS then press # 3
Full sequence: Press [MSG] or [CALL BACK] button,press 6 for MS then press # 3

I and presuming Message/Call Back, Speed * * to delete all the lines above

Since it looks like you have 6 external Voice Mail messages, I wonder if the mailbox is deleted the phones do not realize it so you have to delete the notifications yourself.
I would presume if the mailbox was alive and you deleted the messages it would populate the VM column.
