Blinking digital phones

Hello guys,
So I have this weird error I get ... When I plug a digital phone (very old Siemens opti) I get blinking LEDs... Even after I changed the phone pairs of wire with new one/also changed phone this error come back later on...
Usually as far as I know digital phones either work or don't... What could cause it ?

Are you sure you've plugged it into an SLMO/SLMU port, not an SLMA ?

And don't forget optisets don't work in V10 and above, so the phone will probably enter a reboot loop.

I'm sure it's on SLMO card... knowing it used to work... Furthermore if I plug at source ( dispatcher right after cards it works perfectly fine)
While knowing I changed pairs main wire (beside others already working fine) and get same error can I assume that whole wire of 32pairs is defective?

It sort of sounds like the pair is also bridged elsewhere, like to another jack. But if you changed pairs and simultaneously ruled that out there's some other reason.

This blinking is not like the one when you wire analog line to digital phone it blinks with time delay not repeatedly... As if it goes off and come back and repeat

On what system are you connecting the phone?
What type of 'Siemens opti' are you connecting? OptiSet E? optiPoint 500?
Does it also happen when you connect the phone directly on the pabx (to rule out the wiring).
Does another phone work okay on the same port (so the phone is defect)?
Try the phone on another port.

We are using hipath v3
I did changed port yet same issue ...
If I connect it directly to PABX it works fine.
Seems main cable yet not sure about it

Something caught my attention extensions having same issue are on the same self 1-3-x with 1/2 tm2lp card not working could it be the problem? What does tm2lp card do ?

TM2LP is for analog trunks. A TM2LP in defect state should not affect an SLMO. Besides, the Optiset you said works when connected directly to the 4K. A reseat or res-bssu may bring the TM2LP back in service, you could check the config is right (the ADD-BCSU), it should be the same as the card on its left. But it's not the problem here.

If the optiset works directly on the 4K but not at the other end of the wiring, I think you have found the problem.
