Blind transfer not working 1

I have an IP Office 500 V2 A, v11.1.0
Users say they were previously able to perform blind transfers, now the calls are just put on hold. The only option is do an attended transfer where the second party must answer and then press transfer again to complete.
If I'm not mistaken transfer on hangup is a default feature, so I'm at a loss as to why this stopped working and where to go to resolve the issue.
Any assistance would be greatly appreciated.
 
Hi, have you updated the release? Plus do you have SIP voip phones?
let me understand the problem clearly, when you do the blind transfer to the other side is it silent? this thing happened to me

I haven't updated the system.
There are no J series phones or SIP phones used on this site.

Really! When prompted, but only when prompted, you're going to tell us what phones they don't have but still not tell us what phones they do have! Nor are you going to say anything about the dialing sequence they use to try and make blind transfers, whether the issue is on internal or external or both types of call, types of lines involved, etc.

Stuck in a never ending cycle of file copying.
 
The blind transfer are done using the transfer button, where the user will press transfer, dial the other extension then hang up, which would automatically transfer the call before. Now the called extension stops ringing and the call is placed on hold where the transfer originated. This occurs on both internal and external calls. They only have 1403, 1408 or 1416 phones. The types of lines are analog from fixed wireless devices.
If any other information is required, I would be happy to provide.

I bet they don't hang up but hit the drop button instead like a lot of people do when they want to end a call especially if they have a headset.

Make sure you do it yourself and not trust the users word on how they do it.

I have tested it as you all have indicated and I have to complete by pressing transfer again. I have advised the users of this as well, unfortunately for me they insist that this is not how they are accustomed to doing transfers and that it is a problem that needs to be fixed.
