Bix block problem

Has anyone seen a bix block go bad? I haven't used bix that much, I prefere 110, but I got a new customer that has bix. I pulled up a jumper to do a move and when I punched down the new on, it didn't make connection. After a real headache of trouble shooting it was the block that was bad. Fortunately I had a spare block laying around and there were that many station cables on this block and they all had spare cable. I changed out the block and everything worked find, I put the old block back in place and it failed.

Had anyone seen a bix block fail like this?

Yes when someone didnt have the BIX tool and they used a screw driver or something and it ruined the pins.
 
Yes, I have had to replace QCBIX1As in the past for the same reason. Because someone not familiar, or did not have the right tool, damaged the connector. Even though others make BIX bits for their handles, I prefer the QTBIX16A tool. It works every time.

I have been using the BIX cross-connect system since 1989...
