Best Way to split one jack to support 2 partner phones

I have a cable run currently supporting 1 Partner 18D phone. I would like to be able to support 2 phones on this cable. What is the easiest way to accomplish this. One end is terminated into a Cat5e Patch Panel (All 4 pairs) and the other end has a RJ45 (Pair for Pair).

If I Use the Blue/Orange for phone 1 and Green/Brown for phone 2, where would the Colors match to on the RJ45's? Also what would need to be done on the patch panel side?

I figured the Blue to Blue and Orange to Green on RJ45 #1, and Green to Blue, Brown to Green on RJ45 #2. But then what do I do on the patch panel? I figurs making some kind of "Y" connector to pull all 4 pairs off the panel and have the Y go to 2 RJ45's wired the same way as above?

You need to make a splitter for both ends, using a cat 5 patch cord and 4 jacks.

Cut the cord in half. Terminate the Blue and Green pairs on one jack, following the color code.

Terminate the Orange and Brown pairs on the other jack, substituting Orange for Blue, and Brown for Green.
