BCM50 Conference Calling Issue

We have a customer reporting a problem with conference calling on their BCM50 3.0. System is configured with PRI T1 and DSM32. A couple of months ago they were having a problem where when they conferenced two outside callers together with one inside caller, only the two outside callers could hear each other. We power cycled the system, and this fixed the problem. Now, the problem is happening again. Does anyone know if this is a known problem and if there are any patches to fix it, or have any other feedback?

The last few bcm50 3.0 patches are more for voip trunks and Ip phone firmware but is the latest su installed?(SU.System-53.200804) I would strat with that if it's not allready installed.
